Student Insurance Forms

All students are required to have health insurance while at Colgate. You and your parents should complete the online waiver form each summer providing information about your coverage which we keep as part of your health record.

For the 2012-2013 academic year, please visit www.gallagherkoster.com/colgate to waive or enroll. Please note the deadline for waiver/enrollment submission is 09/07/2012.

Students wishing to make changes in their insurance coverage during the academic year should contact the student accounts office at 315-228-7812.
