Report of Sexual Harassment, Sexual Assault, Dating/Domestic Violence, or Stalking

First, if you have experienced sexual harassment, sexual assault, dating/domestic violence or stalking, Colgate wants to support you in any way we can.  Whether or not you file a formal complaint, you are entitled to support services, some of which (like Haven, or the counseling center) are confidential.  See https://www.colgate.edu/about/campus-services-and-resources/sexual-violence-support for a full description of resources available.

This form may be used to report information to Colgate administrators related to alleged violations of Colgate's Student Non-Discrimination Anti-arassment and Sexual Misconduct Policy, which prohibits sexual misconduct of various types, including sexual harassment, sexual assault, sexual exploitation, dating/domestic violence, stalking, and retaliation. The online form is received by the Title IX Coordinator, who retains the report in the Office of Equity and Diversity and shares it with other university administrators on a need-to-know basis.

Except in cases of clear danger to the community, the information provided in this form does not trigger any investigation or adjudication of the alleged behavior unless the reporting party indicates that is the desired outcome. If the reporting party provides their name and contact information and indicates a desire to be contacted, a Colgate administrator will reach out to provide resources, support and information on options and next steps.

Background Information

You may submit this report anonymously if you do not wish to provide a name or other identifying information at this time. However, the nature of anonymous reports makes responsive action more difficult and, at times, impossible. The nature of the report, the date, and location of the incident are the minimum required information for federal reporting purposes. If you wish to receive a copy of this report for your records, you must provide an email address below.

Involved Parties

Please list the individuals involved (including yourself), providing as much information as you are willing and able to share. You are not required to provide the names of the individuals involved, but if names are not included it limits the University's ability to take responsive action. Filing an anonymous report ensures that information is recorded in the event the survivor decides to file a complaint at a later date, and also assists the university in compiling accurate crime statistics.

Note: A "Bystander" is an individual who witnesses or learns of an incident, but is not directly impacted as a victim or survivor of this incident.
