Students

For students participating on other Colgate-sponsored travel (domestic or international):

Unless advised otherwise, please submit the three forms below to the campus sponsor of your travel program one month prior to departure.

• Student travel plans form (PDF) - complete and return to your campus sponsor
• Confidential physician's report for independent travel (PDF) - STUDENTS: Please complete questions 1-10 yourself and return the form to your campus sponsor. DO NOT TAKE THE FORM TO YOUR FAMILY DOCTOR OR TO STUDENT HEALTH SERVICES.
• Acknowledgement of risk and responsibility for small group and independent travel (PDF) - complete and return to your campus sponsor

Upon receipt and review of your completed paperwork, students traveling abroad will receive an International SOS card.
 

For faculty, staff, or department supporting small group or independent Colgate-sponsored travel (domestic or international):

• Please collect the students' forms (the three forms above from each student), save a copy and return the originals to Off-Campus Study one month in advance of travel. Off-Campus Study will forward the physician's report to Dr. Miller for her review. For those traveling abroad, this will ensure that students are covered under the International SOS program.
• Faculty or staff members who will be traveling off campus or working on site with students should also complete the Faculty/Staff travel plans form (PDF) and return to Off-Campus Study one month in advance of travel. For those traveling abroad, this will ensure that the faculty or staff member is covered under the International SOS program.
